What happens if a doctor leaves Optima Health?
If your Plan doctor leaves the network, Optima Health will notify and assist you in finding a new doctor or facility. If you are in active treatment with a doctor who leaves the network you can request to continue receiving healthcare services from the doctor for at least 90 days.

The formulary is a list of drugs that are covered under your Plan. Most Optima Health plans have a four (4) tier formulary.
