eviCore provides transitional care services for members (except those described above under ‘Excluded Members’) discharging from the hospital with HHC services. Members are managed by the eviCore Transitional Care Program for 90 days post-hospital discharge. The Transitional Care Program facilitates member support based on identified risk factors.
