Forest Hill Healthcare Center is a state-of-the-art Post-Acute Care and Long-Term Care nursing center located in the Historic Forest Hill section of Newark, New Jersey. Surrounded by residential neighborhoods bordering Branch Brook Park, Forest Hill Healthcare Center is Newark’s best kept secret!
Post-Acute Care
Forest Hill's Post-Acute Care Unit is a dedicated recovery and rehabilitative setting for patients recuperating from an injury, surgical procedure or serious illness.
Long Term Care
Half of Forest Hill’s 120 beds are devoted to long-term care. Long-term care is for patients who can no longer live on their own and require assistance or supervision with medications and activities of daily living.

Established in 1995, Forest Health and Rehab is a 97 bed skilled nursing and rehab community located in Lynchburg, VA, which many may know as “Hill City” or “The City of Seven Hills”.
What is a progressive approach to home?
Our Progressive Approach to Home (PATH) is a unique, transitional care model that utilizes evidence-based practices and medical models. We use this program and transparent communication while working to return patients to their homes as quickly and safely as possible and reducing their number of hospital visits.
