The Importance of The Step Up and Step Down Variations

Step ups and step downs are both amazing exercises for prehab and rehab purposes alike.

The Step Up

The step-up explicitly demands single leg postural stability and coordination as well as dynamic control of the pelvis and trunk. The step-up also serves as an adequate stimulus to foster endurance and strength of the lower extremity and can easily be progressed or regressed depending upon the goal or ability of who is performing this exercise!

Cross Over Lateral Step Up

A recent systematic review by Macadam et al in 2015 highlighted cross over lateral step ups. They found that the cross over lateral step up produced one of the highest Gluteus Maximus muscle activity (103 +/- 63.6%) in their review.

Yoak Step Ups

If you want to challenge your balance, you need to add demands to the task that decrease your stability. Perturbations are an effective way to add instability to your exercise. Perturbations are awesome because they provide constant variability. Research has shown the variability of practice can lead to greater skill acquisition and retention.

Getting Stronger? Now On To Step Downs

Step downs are a staple in our lower extremity rehab progressions and for good reason. You can isolate/bias one leg without any “cheating” using the other leg like on squats, deadlifts, or the lunge variations we have discussed.

Posterior Step Down

With knee rehab patients, I will typically start with backward step downs as the demand is less on the knee and more on the hip. The trunk will flex forward naturally to keep your center of mass on the box, thus utilizing the glutes/hip more in addition to shortening the lever arm on the knee.

Lateral Step Down

Remember, due to the step down occurring more in the frontal plane than the sagittal plane, the sideways step down exercise will place more demand on the hip abductors. The big keys here are to ensure your hips stay level and do no drop. Imagine wearing a belt and keeping it straight the entire time. Furthermore, do not reach to the ground.

Needs improvement

Wants you to work extra hours but doesn’t want to offer bonus to over worked staff. Can’t even get a simple thank you lunch for staff but as soon as management has a meeting or Q&A they cook or provide lunch.

Understaffed , under payed

Only care about themselves , underpaid and understaffed . Everyone is related there and only look out for themselves. They train you for a day and get mad when you ask questions

I love helping the residents

I would not bring my parents there if they were alive. They are under staff, cover up stuff and heartless nurses. The cnas sit in the residents room while playing on their phone and leaving the residents waiting to be taken care of.

Horrible and heartless nursing management

I was at this building for many years it was slowly getting worse but as of the end of July everything hit the fan after our one good nurse manager left. There were a lot of good nurses and CNAs.

Not at this time

The DON was clueless. Whenever I asked for assistance during training, she told me to ask someone else. Many things occurred that left me feeling undervalued and unappreciated by the DON and ADON.

Do not work here

Does not pay well at all! Management sucks. Administrator does not care about the nurses. Company is cheap on supplies. Benefits are awful. Only good thing is earning PTO.

Great work place

A very wonderful company to work for. The Administrator is wonderful and does an exceptional job for the facility. The Management and staff are great people. I currently have been here for two years and I love it.

Supporting Staff

I love this job. Just like every job, it has its ups and downs. But staff get through everyday together. There is new management that has new ideas that works perfectly

Productive and fun place to do my internship

The staff are very welcoming an encouraging. My supervisor was very knowledgeable and did a great job at mentoring me. The atmosphere was always fun, positive, and energetic.

I shadowed there while receiving field hours for my college program

This facility was a great place to shadow. They support their residents any way that they can in order to make end of life care enjoyable. There were always activities for residents to participate in, of course with plenty of staff support.

Okay

I was only a volunteer for a couple days. I just helped with entertainment for the residents there. I learned about how to handle different situations with different types of people.

Like any other nursing home I have worked for.... understaffed

The patients were the best part of my job. The work is overwhelming and I never knew where my nurse supervisor was. She was supposed to help me because I spent 95% of my day on the hall passing meds. Everyone always came to me because I was always on the hall and visible.

Really fast paced environment depending on your roll, managers take on favorites. But if you get the hard assignments as a CNA you learn a lot if you look at it positively. If you like working hard then this is the job for you.

Five points health care

It’s a good environment to work for, it’s flexible with your schedule, it has maternity leave and a 401 k, it gives you plenty of pto and it’s a good team to work with.

I like my job

They are awesome people to work with and I love the people I work with at night time! We have an awesome crew and we all get along very well and we work as a team
